would be nice if they all unlocked once the requirements were reached 
I am a level 65 411 Item Lv Priest on Thulsa server, never played pvp and completed each dungeon up to and including Lilith Keep at least once, yet according to my psn trophies ive never reached level 60 and never completed Labrynth of terror, Ebon Tower..ect all way up to Marcellus Catacombs..soo yea get that fixed soon please as Tera is just a shell of what it once was and at the moment trophies are the only reason im playing the game on ps4..

Basically, after you do the Sky Garden dungeon, the trophies no longer want to work. You can get one of the boss trophies to unlock if you have the in-game achievement and redo Sky Garden. All dungeons, levels and the Arun World bosses trophies are bugged after Sky Garden though.
