At that point you are prioritizing each move based on the mobs health or how soon you will have to dodge. Rather than starting an innate chain and pressing triangle until I have to dodge.

One known cause is ignoring the chain on screen in favor of another. For instance if you hit over power and go straight into a whirlwind instead of hitting tenacity the whirlwind will not lead you into overhand strike.
Doesnt matter the skill. If for some reason you used whirlwind right after heart thrust or knockdown strike after whirlwind the overhand strike wont pop.
Thats my only problem with the chains is you have to follow each one for it to work. I dont want to use overpower-tenacity-dash everytime. But if I go overpower heart thrust ohs doesnt pop.
