This game has the worst performance of any game I've played in recent history.
I've applied every 'fix' suggested in every thread I can find but in most dungeons (and ESPECIALLY in the open-world guardian missions) my FPS is so bad it's nearly unplayable. What gives?

Your setting is overkill for tera, you need to be specific in what your settings are.
Guardian Missions will generically force you to lag because the main thread handles almost everything. Your graphic engine has absolutely no impact on how much graphical resource it needs to render everything, because it doesn't require as much as you would expect. But imagine
Also I hope your Windows OS is a fresh install and not just a OS upgrade, because it is the main reason why many players struggle with FPS issues (driver settings still on Windows 7/8)
In case you are wondering I have very few issues at medium-high settings, with a GTX1060 6gb and an i5 7600 (no K) tower, you just have to get used to the fact that no CPU is able to handle the instructions of 30 different characters (including costumes AND classes which are known to cause lag such as Gunners and Reapers) at its proper speed on a single thread just because that is too many instructions in one sitting.
I used to do this as well when building. I set my fastest drive for the OS, and the second fastest as page file. The rest were storage.
Killing memory leak THE HARD WAY
Runs exactly as good/bad on my HD as it does on an external USB 3.0 HD, and exactly identical FPS as my SSD. Only load times are faster on the SSD and both HDs performed identical (since USB 3.0 caps the drive's speed anyway). May I also add that TERA doesn't take a gigantic amount of small files, rather medium/big ones as the sequential speed read makes a nice difference on loads, but the SSD didn't exactly destroy the HDs.
I will definitely not be surprised as well. TERA is DirectX 9, and older cards are notoriously made for that, quoting yet again my GTX 550 Ti here with OK FPS everywhere (20 Velika, 40 dungeons, 8 CS gate huehue, 30~40 during CS match, 40-80 open world) (edit: that on MAX graphics, save for Light Enrichment set to 1, screw that blur) and flat 60 (or even the weird 80 cap) on absolutely anything including the gigantic mass of player events when set to low.
Windows 10 Pro with 8GB of RAM AND A CONTROLLER by the way.
It's weird to recommend an old graphics card to run a game, but hey, it's TERA. Having 2 PCIe slots, you could actually keep the new card and drop an old one there and make the game run anything but TERA on the new one with only PhysiX on the old one (for like 10 games that use that in-hardware, TERA is NOT one of them no surprise here), and then configure TERA to run ONLY on the old card.
Why should they bother to optimize the game when there are so many people stating the game is running just fine? Unless people can get on the same page, nothing will be done.

CPU-wise i'm currently using i7 4790k and if i set it to 75%~ use (2,8ghz~ iirc) things can get pretty [filtered] real quick, but pumping it up to 3.8+ literally doubles performance, even then, when there's too many players you'll get grey blobs and garbage fps, big open-world events/rally bam i'm usually at 5~10 fps even with CPU at 4.2+. All PvE content is 100% smooth though including old HH30.
As to game options, setting min or max graphics barely makes any difference to me on FPS as well, while s1engine changes are HUGE for me and most people if you know what lines to edit and play around with them.
They need to re do the UI, that is the FPS killer.

They actually don't.
One of the reasons why they look less laggy in Ktera is because they have lower ping. Oddly enough ping does affect FPS in some way in this game (not by much but it is noticable), which I assume is based off the overloading of instructions to the main thread. Also they are typically using dated hardware which Tera just works better on honestly.
