Superior Glyphs

Where to obtain superior glyphs (yellow triangle at the top left corner), like the attack speed increase glyph of Gunner's Blast?

Also out of curiosity, does each class have and only have one superior glyph?

    It's given to you in the Apex quest line (requires i439).

    And yes, they all only have 1

    To get Superior glyph you need complete the apex quest, it unlocks at ilvl 439
