Not having a live test server isn't the issue, as they mentioned it works in QA, it just doesn't work on the live servers for an unknown reason.
Why is it so hard to understand a live server and a test server are different. A live test is a game server that is identical to the game servers and only used to test new software/patches. Normal test servers differ in setup and actually don't have exactly the same setup or load. Also test servers are in house 90% of the time and a live server would be on the same network as the game servers.
It isn't hard to understand; the point is that even that may not have solved the problem. Even "live test servers" have to have subtle differences to keep it isolated from the rest of the servers (because it'd get the live patches before the main servers, and require a different client). EME actually does have servers like this that they use for major patches (their "qa2" branch) that are at the same datacenter as their regular servers, but the client isn't cross-compatible. (We usually call these "live test" servers "staging servers" in my work, and yes it's clearly distinct from the internal dev/test servers. But sometimes some issues only show up on live, even still.)

Just because they got it, doesn't mean it would work flawlessly for us... Even other games see different issues for different regions from time to time... Any slight difference between NA and Eu could easily cause any problems.
Having a test server (even one that is public) doesn't mean all issues will be found. Look at LoL, They had a public beta server for a while and bugs that didn't exist there would pop up on the live servers.
