The thing when the boss attacks backwards, it happened to me too, but not in sshm or ssnm, but in dfhm. I don't know how it happened, but Vera kept attacking me while looking the other way, was confusing and funny.

As Nopi said, the issue lies with the Ninja Skill One Thousand Cuts. For some reason this skill makes the BGM fade in an out, and is a known issue that Spacecats confirmed to me on a PM and they will work on a fix.
